For Diplomates
Active ABCC diplomates may use the diplomate login portal to generate a letter of verification for their records. One can be produced under the “Certifications” tab in your diplomate portal.
For Credentialers
For credentialers, please complete the form below to submit a request for a letter of verification confirming board certification status of a diplomate. Please ensure you complete all required fields and provide a signed consent form from the diplomate for us to release the requested information. We process all verification requests in a timely manner. Please expect to receive a verification letter within 2-3 business days.
